Interdisciplinary Model of Care

At Tavares Pediatric Center our interdisciplinary team of therapists, nurses, and medical professionals work with the parent(s)/guardian to develop an Individualized Person-Centered plan of care to include short and long term goals and objectives, to maximize each resident's potential for growth and well being.

  • 24 hour/365 day ICF-IID Skilled Nursing Care
  • Respiratory Therapy
  • Case Management Services 
  • Physician on-call 24 hours 
  • Education Services
  • Onsite Day Program for TPC residents
  • Nutrition/Dietary Services
  • Occupational Services 
  • Physical Therapy
  • Program Coordination
  • Recreation/Leisure Activities

  • Resident's Parent/Care giver education and teaching

Criteria for Admission

Given the specialized care provided at TPC there are specific Admission criteria such as:


  • The child must receive all nutrition and hydration via a G-Tube/J-Tube
  • The child must meet criteria for an ICF-IID level of care.
  • The child must be non-ambulatory.
  • TPC prioritizes admissions based on need and urgency of the case
  • We ask that you have the c

  • The child must be a Rhode Island resident and the age of 17 or younger.
